If this is your first time injecting anything, the cheapest programme is not automatically the right one. What matters in month one is whether somebody answers when a dose goes badly, and whether slowing down costs you money.

We ranked these on the two things that decide whether a beginner stays on treatment: how the dose ladder is priced, and how reachable the clinical support is. Cost still counts — it is the heaviest single weight in our rubric — but it is not the only thing in the scale.

Why flat dose pricing matters so much at the start

Tirzepatide starts at 2.5 mg, which is a starter dose rather than a treatment dose, and climbs through 5, 7.5 and 10 mg. Semaglutide does the same from 0.25 mg up to 2.4 mg. On programmes that charge more per rung, every escalation raises your bill — and, worse, going *back down* to settle side effects can feel like wasting money.

A flat rate across the dose range removes that pressure entirely. Coreage Rx holds one price from 2.5 mg to 15 mg; Eden holds its medication price flat too, though a $99 monthly membership renews behind it.

1. Coreage Rx — Best overall on true monthly cost

Dose increases are reviewed rather than applied automatically on a calendar, which is the right way round. If month two has been rough, there is no financial pressure to push on to the next rung just to feel you are getting value, because the next rung costs the same as this one. That removes a genuinely bad incentive that exists at most of the dose-priced competitors.

You step through the standard ladder with clinician review at each stage, and the bill does not move. If week three of a new dose is rough, staying put for another month costs nothing — so the conversation with your clinician stays clinical rather than financial.

Conventional escalation, reviewed by an independent clinician rather than applied on a timer. The price holds at every dose, so there is no financial nudge toward stepping up before you are ready.

4. Ivím Health — Unlimited clinician access

Weekly adjustment based on how you are actually doing, rather than a fixed four-week escalation. If a dose is not sitting well, the change can happen in days rather than at the next scheduled step. That is the single best answer to the rough fortnight after an increase.

5. Mochi Health — Insurance concierge support

Intake is thorough, which slows sign-up and improves what happens afterwards. Dose decisions are made with more clinical context than most of this market bothers to gather. If a dose increase goes badly, there is someone to tell who will actually adjust the plan.

Conventional escalation. The monthly plan genuinely cancels at any time, which matters more than it sounds — a meaningful share of people stop within the first eight weeks, and being locked in makes that decision worse.
